Incident Investigations 

We know all too well that Trusts can identify some incidents that would benefit from an external investigator to ensure impartiality. Having both completed HSSIB investigation courses and having detailed insight into the coronial system, we can assist/manage/investigate sensitive deaths that have occurred in your trust. We will work with your Patient Safety team and clinical teams to thoroughly investigate the root cause and liaise with the coroner and your legal team.
